Malignant Carcinoid Tumor of the Duodenum




Abstract
The duodenal carcinoid tumor is very rare case and a more clinical significance that other carci
noid tumor of the gastrointestinal tract for there anatomical important position.
A case of carcinoid tumor of the duodenum was encountered recently at Department of General
Surgery, St. Mary¢¥s Hospital of Catholic Medical College.
The patient had been suffered from dull discomfortness on the right upper quadrant abdomen
intermittent high fever with chilling sensation, and weight loss for 40 days.
On physical examination, a hard, not movable, and egg sized mass was palpated on the right upper
quadrant abdomen, but there was no tenderness.
Under the impression of malignant neoplasm of the duodeum, pancreas or biliary system, the
exploratory laparatomy was performed and pancreaticcducdenectomy was performed for resection of
tumor of the 2nd portion of duodenum with invading the head of pancreas and intraduodenal portion
of the common duct.
The diagnosis was dependent on the histologic pattern of small round or cuboidal cells with uniform
dark nuclei forming nests of island and cord.
The preoperative amount of 5-HIAA was 142.6§·/24 hrs. urine.
The preoperative condition of the patient was very good and discharged on 25th operation day.
